On-Line Registrations will be accepted through March 21, 2008

This year’s screening will be held on:
SATURDAY, APRIL 19
BEATY WARREN MIDDLE SCHOOL
6 AM TO 10 AM

Blood samples will be drawn and analyzed for 30 separate characteristics.
Warren General Hospital will be coordinating all lab evaluations.

The base cost of the screening is $35 per person. Prostate screening may be added for an additional $25 per person. This test is generally recommended for males 40 years or older.

Payment may be made by cash or check, payable to “Kiwanis Club of Warren” upon your arrival on April 19. You will not receive a response to this on line registration, but will receive a reminder post card prior to the BLood Screening.

FASTING FOR THE 12 HOURS PRIOR TO YOUR TEST TIME IS REQUIRED. CONSUMPTION OF WATER, BLACK COFFEE OR PLAIN TEA IS PERMITTED.

We are sorry, but on-line registration for the Kiwanis blood screening is no longer available. You may manually register on Friday, March 28th and Saturday, March 29th , from 10:00 AM to 9:00 PM at the center court of the Warren Mall.
